Mit beiden Konzepten soll letztlich eine positive Knochenbilanz erzielt werden. Ermutigende kurz- und mittelfristige Ergebnisse bedürfen noch der Absicherung durch kontrollierte Langzeitstudien (132).
Als adjuvantes Therapeutikum wurden Bisphosphonate beim Hyperparathyreoidismus eingesetzt (72). Ihr Effekt ist jedoch bei diesem Krankheitsbild begrenzt, da nur bei 4 % der Patienten schwere resorptive Knochenveränderungen vorliegen (179). Eine Senkung des Serumkalziumspiegels gelingt unter Clodronat ebenfalls nur inkomplett, da beim Hyperparathyreoidismus die Hyperkalzämie in erster Linie auf eine gesteigerte renale Reabsorption zurückzuführen ist (36). Von den genannten klinischen Einsatzmöglichkeiten hat die Verwendung von Clodronat als antiosteolytisch wirksame Substanz bei Knochenmetastasen die größte Verbreitung gefunden. Aktuelle Empfehlungen dehnen die Indikation aus auf präventiven Einsatz des Medikamentes noch vor Nachweis von Skelettmetastasen, wenn ein Primärtumor mit hohem Risiko einer ossären Aussaat vorliegt (z.B. Mamma, Prostata, Lunge; 93, 95). 1.3 Knochenregeneration (versuchsrelevante Grundlagen) Beim Knochenumbau spielen sich Resorption und Formation innerhalb sogenannter "bone remodeling units" (BRU) in einer festgelegten Sequenz ab (59). Die Aktivitäten der Osteoklasten und der Osteoblasten finden nicht unabhängig voneinander statt, sondern gekoppelt ("coupling"). Dies gilt sowohl für das remodeling, dessen Ziel der Erhalt der Makroarchitektur ist, als auch für das modeling, dessen Ziel deren Änderung ist (19, 61). Der Versuch, äußere Einflüsse auf die Aktivität eines der beiden Akteure (Osteoklasten/ Osteoblasten) isoliert zu bewerten, wird erschwert durch die möglicherweise den Gesetzen des "couplings" folgende Mitaktivität des Konterparts.
